PRINCIPAL PURPOSE: To record the essential background information of a 28th Infantry Division veteran, a veteran's family member, or an associate of a veteran, who may have historical significance to the United States Army.
ROUTINE USES: This form will become part of the U.S. Army's historical collection and may be used as a basis for further communication with the veteran. The information on this form will be used by historians and researchers as a record of a veteran's history and recollections.

INSTRUCTIONS: All veterans' experiences are important. By filling out this form, you are educating every historian, researcher, family members and soldier who utilizes the 28th Infantry Division Association's historical resources.
Many veterans have already shared their stories with the 28th Infantry Division Association by sitting for an oral interview. To truly capture the breadth of our veterans’ experiences, and to protect and preserve future generations, we need YOUR story.
Many of our veterans and their families choose to share their stories by making unconditional gifts of their materials to the PA National Guard Military Museum. Their personal papers and historic objects are registered in the donor’s name, and all items have a permanent, secure, state-of-the-art, climate-controlled home to preserve them for future generations. Items are available to students of all ages, and the general public, honoring your service as a respected part of our nation’s military heritage. You can contact the Museum at (717) 861-2402.
